Registration for the Midlands Hospitals' Choir Christmas Celebration Concert
It helps us greatly if you can pre-register for the Christmas Celebration Concert.

Once you have filled in your personal details (highlight any recent changes) there are two ways you can complete registration:
1. Post the completed form along with your membership fee (£35) to Mrs Janet Webley (Chairperson) at the address on the form.
2. Bring the completed form and membership fee and register after this year’s Annual General Meeting on Thursday 25th September 2008 from 8.00pm.
